To make this guide i used the files of the game itself so this is actually inside game info. You can see in this "list" exactly in which acts can formulae drop and you should only take a look at the High or Highest chances to drop acts and go farm the formulae there, the rest is just for info purposes.
The "chances" in quotes refers to "weights" in the ingame database, this is for those who know the inner workings of the game engine, players shouldn't pay too much attention to this. What players should actually take a look at is only the Highest or High chance and go to those acts to farm the formulae.
The comparison of each item is about the item dropping in different acts, it is not in correlation to other items in the same loot tables. So, Highest chance / Low chance means that in the act that has the "Highest chance" wording, you have more chances to get the item than in the act with "Low chance".
Atlantis dlc brings no new formulae to the game.
Ragnarok formulae drop only at Act 5 of all 3 difficulty levels and Act 1 Epic/Legendary (not in Act 1 Normal).
The list is in Alphabetical order per tier. There are 3 tiers: Lesser Artifacts, Greater Artifacts, Divine Artifacts.
There are 84 artifacts overall. There is a side quest in Ragnarok dlc which gives as reward a Lesser Artifact which has no completion bonus. That reward is always a random artifact and always a Lesser artifact. It will never be one of the next 2 tiers.

There are 6 new artifacts the new dlc brings. They drop only in Legendary difficulty and the highest chances to get those are in:
